How to Nominate

1. Complete the nomination form which can be downloaded by clicking here.


2. Nomination Forms should be completed and returned by close of business on Friday January 27th 2012. Nominees should be profiled only by the completion of this form. Please do not send any additional information at this stage as such material cannot be considered by the Selection Committee.

3. The Selection Committee will shortlist a selection of nominees in each category.

4. A successful award winner will be chosen in January 2011 for each category.

5. There will be an awards ceremony in the Oriel House Hotel for all candidates.

6. The successful candidates will each receive a special commemorative award.
